This is a piston retaining nut which has come loose from a hydraulic cylinder. The nut was destroyed by the piston before the rod assembly left the cylinder barrel. When a hydraulic cylinder comes apart it can be a catastrophic failure. Correct nut torque may be only part of the full procedure.
